Registration for new students K-12 can be completed at the Lakeview Jr.-Sr. High School Building, 3744 83rd Street. To make an appointment, contact Jillian Cuda at 564-8518.
Documentation required at the registration appointment is as follows:
1) State certified birth certificate – must be state issued with seal, no copies will be accepted
3) Two proofs of residence – 1) current bill with parent(s) name and a physical address AND
either a lease agreement, property tax statement or mortgage statement. (or administrators approval)
State law requires that prior to the entrance of a student into the beginning grade, or in the case of a transfer from an out-of-state school to any grade, physical and vision exams must be completed. As a substitute for such evidence of a physical or vision exam, an objection in writing (signed and dated), should be presented to the registrar. Physical and eye exam forms need to be returned to the registrar at Lakeview Jr.-Sr. High School at 3744 83rd
Street, prior to the child's first day of school. All immunizations will also need to be complete and proof given to the registrar before a student will be allowed to enter school.
Students registering in grades 9-12 will need a transcript from their previous school prior to registering for classes or attending school. Once we receive the student transcript, the incoming student will be assigned his or her grade level. If a transcript is not received in a timely manner, administration may make the decision when a student may begin school.
