Lakeview Community School District Student Registration
Registration for new students K-12 can be completed at the following link:

Once the form has been completed please bring the Required Documents to the Lakeview Jr.-Sr. High School Building at 3744 83rd Street, Columbus, NE 68601. If you have any questions, feel free to contact Nicole Osborn at 402-564-8518. 
 
Required Documents:
  1. State-certified birth certificate – must be State issued with the official seal. Copies will be made at the District office.
  2. A copy of up-to-date immunization records.
  3. Two proofs of residence – 1) current bill with parent(s) name and a physical address AND either a lease agreement, property tax statement, or mortgage statement (or administrators approval).
  4. Additional documents listed below.

State law requires that physical and vision exams must be completed prior to the entrance of a student into the beginning grade (Kindergarten), or in the case of a transfer from an out-of-state school to any grade. As a substitute for such evidence of a physical or vision exam, an objection in writing (signed and dated), should be presented to the registrar. Physical and eye exam forms must be returned to the registrar at Lakeview Jr.-Sr. High School Building at 3744 83rd Street, prior to the child's first day of school. All immunizations will also need to be completed and proof given to the registrar before a student will be allowed to enter school.

Students registering in grades 9-12 will need a transcript from their previous school prior to registering for classes or attending school. Once we receive the student transcript, the incoming student will be assigned his or her grade level. If a transcript is not received in a timely manner, the administration may make the decision when a student may begin school.
